OUR PROCESS
Within 48–72 hours of receiving case materials, we conduct an initial review of the records to assess scope, identify the key clinical issues, and determine the appropriate analytical framework. You will receive a clear summary of what the records contain, what clinical questions need to be answered, and how Evidentia proposes to structure the engagement. Nothing moves forward without your direction.
Initial Case Review
Medical records, nursing documentation, diagnostic reports, and ancillary materials are reviewed systematically against the applicable standard of care for the clinical setting, patient acuity, and relevant regulatory requirements. We build a comprehensive chronological timeline of clinical events, identify deviations from standards of care, flag documentation gaps and inconsistencies, and conduct any necessary medical literature review to support the analysis. Every finding is traceable back to the record.
Structured Record Analysis
Deliverables are written for attorneys — not deciphered after the fact. Your findings report includes a structured clinical chronology, a deviation summary with record citations, and clear written conclusions organized for use in deposition, mediation, or trial. We remain available throughout the litigation for follow-up questions, deposition preparation support, and expert witness coordination. The engagement ends when your case is ready, not when the report is sent.

A: All case materials are handled through an encrypted file portal. We do not accept confidential records via email. Do not transmit any PII or protected health information until an engagement agreement is in place.
